Main role is to provide non-medical care to clients, as well as assist them in daily living activities they might have trouble with on their own. Because of the personal nature of the job, certified nurse’s assistant/home health aides are compassionate and enjoy helping others. They help their clients with hands-on care and shopping for food and other household duties, preparing and serving meals and snacks, and running errands.
Assist the elderly, convalescents and persons with disabilities with daily living activities at the person’s home or in a care facility. Duties performed at a place of residence may include keeping house (making beds, doing laundry, washing dishes), preparing meals, companionship, provide respite for the main caregiver, offer encouragement and perform light housekeeping tasks.
Provide individuals and their families with the psychosocial support needed to cope with chronic, acute and terminal illnesses. Services include advising caregiver, providing client education, counseling and making referrals for other services. The social worker could also provide care management and case management services. These interventions are designed to promote health, prevent disease and address barriers to access to healthcare. Coordinates the overall interdisciplinary care plans for clients. Acts as the liaison between client/family and caregiver to ensure necessary care is provided promptly and effectively.
